Maybe because they are not SHOWN doing it. Soldier of Fortune 1 made a point of people using the bathroom. Wasn't particularly tasteful (if you destroy that turd in the african toilet the protagonist makes a cheesy comment), but there once was a time where these things didn't happen, yes. Jet Set Willy was first I think, though that only showed a toilet for the first time.
